| 1) Fill out the MN Confidential Screening and Consent Form and return it to the BHC Registrar |

| 2) Coaches with Expiring certification cards. You need to have them updated before you will be alloud on the ice. |

| 3) Make a copy (front and back) of your current Coaches Card and give the copy to the BHC Registrar |

| 4) Register with USA Hockey as a coach. Here's the link: http://www.usahockeyregistration.com/. Submit your reciept to the BHC Registrar so she can claim you as a BHC Coach. |
