This form is for players that go to school outside of your association boundaries who wish to remain skating with you. It is only for players that played for you last year. NEW players do not qualify for this option.

Mites may play with their association of residence OR their association of their school. If they are new this year they would have to start at their association of their school at Squirts because they again DO NOT qualify for this one time option. We will be going to the MN Hockey board with this issue at their Fall Meeting to see if all NEW mites may make the choice.

I (we) understand that starting with the 2009-10 Season, the Minnesota Hockey participation rule

changes from residency to school attendance for affiliate membership. Our children attended school

outside our affiliate of residence during the 2008-09 Minnesota Hockey season. It is our choice under

Paragraph IV,B,2,d of the Minnesota Hockey Youth Rules and Regulations that our child continue to

participate with our affiliate of residence. I (we) understand that this is a one-time choice that remains

in effect throughout our child's Youth playing eligibility as long as we maintain residence within the

affiliate�s geographical boundary. If we move outside the affiliate boundary, school attendance will

determine Affiliate participation.
